Terms of Service
These Terms of Service ("Terms") govern the use of the online clinic portal ("Portal") provided by
[One Health Clinic Online Incorporated] ("Clinic") for the purpose of accessing and utilizing office, telehealth and home visit services. By accessing or using the Portal, you ("User") agree to be bound by these Terms. If you do not agree with these Terms, please do not access or use the Portal.
1. User Account:
a. In order to access the Portal, Users may be required to create an account by providing
accurate and complete information.
b. Users are responsible for maintaining the confidentiality of their account credentials and are
liable for any activities that occur under their account.
c. Users must promptly notify the Clinic of any unauthorized access or use of their account.
2. Use of the Portal:
a. The Portal allows Users to schedule and manage appointments, access telehealth services,
request home visits, and communicate with healthcare providers.
b. Users agree to use the Portal for lawful purposes and in compliance with applicable laws,
regulations, and these Terms.
c. Users are solely responsible for their interactions and communications with healthcare
providers through the Portal.
3. Telehealth Services:
a. Telehealth services provided through the Portal involve the use of electronic communication
technologies for medical consultations, evaluations, diagnoses, and treatment
recommendations.
b. Users acknowledge that telehealth services may have limitations and may not be suitable
for all medical conditions. Users should seek in-person medical care if necessary.
c. Users understand and consent to the collection and use of their personal health information
as described in the Clinic's Privacy Policy.
4. Home Visit Services:
a. Users may request home visits through the Portal, subject to availability and the Clinic's
policies.
b. Home visit services involve a licensed healthcare provider visiting the User's home for the
purpose of providing medical care, as agreed upon by both parties.
5. Availability and Modifications:
a. The Clinic strives to provide access to the Portal on a 24/7 basis. However, the Clinic does
not guarantee uninterrupted availability due to maintenance, technical issues, or other factors.
b. The Clinic reserves the right to modify, suspend, or discontinue the Portal or any features or
functionalities without prior notice.
6. Intellectual Property:
a. The Clinic retains all rights, title, and interest in and to the Portal, including any content,
software, designs, and trademarks associated with the Portal.
b. Users may not reproduce, distribute, modify, or create derivative works of the Portal or any
part thereof without the Clinic's prior written consent.
7. Limitation of Liability:
a. The Clinic shall not be liable for any direct, indirect, incidental, consequential, or punitive
damages arising out of or relating to the use or inability to use the Portal or any services
provided through the Portal.
b. Users acknowledge that the Clinic does not provide medical advice or guarantee any
specific outcomes or results from the use of the Portal or the services provided therein.
8. Indemnification:
a. Users agree to indemnify and hold the Clinic and its affiliates, officers, directors, employees,
and agents harmless from any claims, damages, liabilities, or expenses arising out of or related
to the User's use of the Portal or violation of these Terms.
9. Governing Law and Dispute Resolution:
a. These Terms shall be governed by and construed in accordance with the laws of
the respective state.
b. Any disputes arising out of or relating to these Terms or the use of the Portal shall be
resolved through arbitration in accordance with the rules of respective arbitration body, with
arbitration to be held in the respective state.
10. Severability:
a. If any provision of these is breeched, we reserve the rights to sever all ties and agreements.
